To enter, log on to www.celebritylivingweekly.com and complete the entry form, which includes your name, address and telephone number, and submit the entry form electronically. Alternatively you can print your name, address, telephone number on an index card deliverable by the United States Post office and send to: Win This Tamsen Dress, Celebrity Living Weekly, One Park Avenue, 3rd Floor, New York, NY 10016. Limit one (1) entry per person or per email address. Entries in excess of stated maximum will be disqualified.
4. Entries must be received by 11:59 PM on Monday, April 17, 2006 for drawing to be held on or about Wednesday, April 19, 2006 at AMI's offices. One prize will be awarded consisting of: 1 Tamsen Dress Approximate Retail Value ("ARV"): $552. Total ARV of Prize: $552.
5. All entries become the property of American Media, Inc, which assumes no responsibility for late, misdirected or lost entries. Winners agree to publication and/or display of their name, address, likeness and entry form to be made at the discretion of American Media, Inc.
